What is a Primary Care Network (PCN)?

Primary Care Networks (PCNs) are groups of local GP surgeries working together in-order to build on existing Primary Care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to their home. 

PCNs typically serve natural communities of between 30,000 to 50,000 people, based on GP registered patient lists. There are currently 1250 PCNs across England. PCNs are small enough to provide the personal care valued by both people and GPs, but large enough to have impact and economies of scale through better collaboration between GP practices and others in the local health and social care system. H&B PCN and H&B Health at a Glance

Hoddesdon & Broxbourne Primary Care Network (H&B PCN) is a collaboration of the four doctors surgeries in the Hoddesdon & Broxbourne area, in Hertfordshire. The network consists of: Amwell Surgery, Hailey View Surgery, The Limes Surgery and Park Lane Surgery. Together the network supports around 42,000 patients and we have around 150 staff working within the surgeries to deliver Primary Care services for our local population.

H&B Health is a new company created to provide support for Hoddesdon & Broxbourne PCN. The company will act as the coordinator for all PCN services which are delivered across the member doctor surgeries within our network. H&B Health will enable the member surgeries to work better together and share resource to ensure all our patients get a consistent high standard of services across the network. The company directly employ staff working collaboratively with the surgeries and will take on new services to support all our patients in the Hoddesdon & Broxbourne area. H&B Health will take the lead on the delivery of COVID and Flu vaccination programmes as well as the Out-of-Hours Primary Care services (Enhanced Access).

H&B Health will support its member GP surgeries in innovative and forward-looking ways of working to ensure surgeries are able to support their patients with reliable, resilient and efficient good quality services both now and fit for purpose in the future.
