Please use the comments below to feedback any issue you have had working with our system partners in relation to the new integrated care pathway. For example patients not accepted for H@H or hf referral but are eligible or on the integrated pathway

We had our third MDT yesterday. We still have no consistency of partners attending which can make things more difficult. If the PCN was not doing this, it would not happen......

On a positive note we have some good patient outcomes so it is worth doing.

We reviewed this. The template is comprehensive. The template for the senior clinician implies that each patient will need to be reviewed and examined by a GP ?

Will this not create an extra level of work ?

Perhaps the name ‘’Senior Clinician’ has implications.

Or should the examination be added to the second page?

Replying to

Senior Clinician just means the senior assessor taking responsibility for a full assessment and findings for the MDT. This assumes a GP, or experienced frailty nurse usually. I understand some PCNs are thinking of other sorts of clinicians such as paramedics. Whoever it is, they need to take responsibility for a good ACP, TEP and ReSPECT form.
